Main purpose (of data processing)	Manage the care and administrative services provided at the hospital as necessary to provide comprehensive medical care, send patients reminders of their appointments or follow-up appointments, conduct internal surveys aimed at improving the quality and management of healthcare, issue proof of care delivery (for non-work and other purposes), and comply with all applicable legal obligations.
Legal basis (for data processing)	The consent of the data subject or patient, or of a legitimate or vital interest, or for the purposes of compliance with legal requirements, at all times in accordance with the requirements set out in Regulation(EU) 2016/679 of the European Parliament.
Recipients (data transferred)	Your personal data will not be transferred to third parties, except in cases of legal obligation, vital interest, provision of the service, or when the interested party gives his consent. Disclosure of data may be necessary for the provision of the service; disclosure shall be limited to those data that are appropriate, relevant and necessary for the purpose of disclosure (principle of data minimisation) and shall be disclosed to the following: (i)    Insurance providers and mutually beneficial associations of which you are a customer. Refusal to consent to the disclosure of data to these entities may be grounds for your refusal to pay for the health care services provided, which may make you responsible for payment for such services. (ii)   Entities that are part of the Atrys Group
Rights (of the persons concerned)	You can exercise your right to withdraw the consent previously given by you for the processing or disclosure of your personal data. Additionally, you can exercise your right of access, rectification, express opposition, limitation of treatment, or request for portability or deletion of your data, doing so in person at theAtrysPatientReception, specifying the right you wish to exercise and providing a photocopy of your ID.
Fountain (data)	The interested party who signs this document or, when the data are not those of the undersigned, these data may be obtained from a legal representative, volunteer or health care provider.
