1. Prices includes doctor's fee and hospital service fee; however, it does not include the Doctor's fee or other services fees out of the package.
2. This package cannot be combined with any other offers, discounts, corporate partner discounts, or promotions.
3. This package is non-refundable in all conditions.

5. Please make appointment 3 days in advance.
6. Do not eat or drink anything except plain water for at least 8-12 hours prior to your appointment.
7. For Vitamin D and Vitamin B12 level tests have turnaround time of 1 day or 24 hours , the department will send the test results through your email once ready.
